If you have been seen by another doctor/specialist or treated in hospital for the condition for which you are seeing Dr Jalland, then please complete the Medical Release Request Form and forward to the relevant doctor or hospital. This will ensure that we should receive all relevant information for your appointment.
If you have had tests completed in relation to your consultation with Dr Jalland then please complete the Results Request Form.
